To get the facts straight:
1) ALL R18 versions (both download AND physical) are shipping from Japan, hence they are censored. Sorry but that's how it is now (that's the price we have to pay for a simulrelease). Unless someone like Mangagamer picks this later, this is as best as we can get. Remember, NEKO WORKS is selfpublishing this, and they have to abide to the shitty Japanese laws regarding pr0n.
2) There is no such thing as a "western version". The release-day versions are the same for all markets, no matter if you pick it from DLSite or if you went all the way up to the C87. It ships with Japanese, English and Chinese.

don't you get restricted access from the 18x contents if you purchase the game from steam?

Nono. The Steam version completely lacks the R18 material (it's stored on adultsonly.xp3, which is completely missing from the Steam release for obvious reasons)
Your best bet is to buy the Steam version, then swap the files of it with the content of the R18 release, edit the commandline parameters to re-enable Steamworks support, and BAM! You can have your cake AND eat it (full R18 version with achievements, Steam Cloud, etc).
So far, dropping "adultsonly.xp3" alone is not enough to reenable R18 contents on the Steam version (in fact, it will break it). Also be aware that the R18 is the same for all markets and sales channels (DLSite/Booth/Gyutto/physical editions), complete including the mosaics - the only odd one out is the Steam version.
